A3 Problem Solving


A3 Problem Solving is a "mini" Root Cause Analysis tool that can be used by all levels of the organization. A3 Problem Solving is used for day-to-day problems that arise on the unit, floor, or department that do not require a full Root Cause Analysis or project team. A3 corresponds to the size of paper (11x17) that the subsequent report should fit on. It includes the following elements:

Business case

The A3 tool and report is a valuable tool in health care because, when performed in the proper scope, it can be completed within 7 to 10 business days; can be shared easily and rapidly with the appropriate staff; can be easily archived; and is usually performed and owned by front-line staff. A3 Problem Solving is also the investigation tool used in the accountability model for Patient Safety Events.
